POTHOLING

SCOPE OF WORK

• Execute contract and provide Certificate of Insurance showing all requisite coverage

• Prepare and submit for approval traffic control plans based upon the pothole locations shown on the attached plans

• Obtain all required permits and licenses

• Call in Underground Service Alerts to all pothole locations

• Surface locate designated utility lines to determine approximate alignments for locations

• Uncover and field verify depth of all existing utility facilities per the final potholing list provided by the city and record all data obtained.

DATA TO INCLUDE:

Pothole Number // Owner // Material

size // depth to top // depth to bottom

• Provide depth to bottom of any encasements that are encountered, trenching if required to complete

• Provide an accurate location of each facility found, which includes pothole number.

• Notify the City when all locations are accurately identified to coordinate field survey at each location

• Prepare a draft pothole report, including summary and results, to submit to the City for review

• Address all comments from the City to finalize Pothole Report and resubmit

• Follow City standards to complete pothole repairs at all locations

• Follow City standards to complete trench repairs at all necessary locations
